Forms & Ordinances

The Town Ordinances have been developed by the Planning Board and approved by the Annual Town Meeting.

The Forms section contains Items such as Building Permit Application, Septic Plan Application, Timber Cut Application. Some of these are forwarded to the State for their approval after the Selectmen have approved the plan. Others stay local. Also included in this section are applications for Personal Exemptions, Credits, and Abatements.

All of these forms are designed to be printed, filled out and returned to the Selectmen's office at the town hall. They are not to be filled in online. At some point we may transition for online forms but not today.

Forms

This is a list of all forms and applications in use by the Town of Sullivan.
There are now two versions of some of the forms. One version is a regular version of the form that is printed out and filled out by hand.
The new version can be filled out on your computer and then either Saved to your computer and emailed to the Town Office or printed and mailed or brought to the Town Office.
Where a new version of a form exists it will be designated by "Computer form".

Here are instructions for the two methods you can use after filling out the form on your computer.

1. To Save the form to your computer go to the File menu in your browser and choose "Save As", and the form will be downloaded to your computer.
You can then attach it to an email and send it to townofsullivan@myfairpoint.net

2.Print the form, by clicking on the Print button at the top of the form. Mail or bring the form to the Town Office.

After doing either of the above be sure to reset the form for the next user by clicking on the Reset button at the top of the form.

If the form is not Reset your personal data will remain in the form.

Ordinances

This section provides information such as the Community Planning Ordinance and Site Planning Regulations. It is provided to answer most of your questions when tackling a new project. Town Codes are developed by the Planning Board and approved by the Annual Town Meeting. Any citizen of Sullivan may submit an idea to the group.

If a particular code, such as Building or Electrical in not available as a Sullivan specific code, it does not mean there is no code or regulation. It means we defer to the State of New Hampshire Codes. The links for many of these are available on the Community / Helpful Links page.
